Clinic name, contact person's name, phone number or email, city, number of branches, selected plan and billing period, and an optional comment.
How the data is used
The PROQYZMET team uses it to review the request, contact the named person, and discuss clinic onboarding. Do not include patient data, health information, passwords, or payment details in the comment.
